Primary Care means all of the health or social care services that you can find in your community, outside of a hospital setting. This includes G.P.s, Public Health Nurses, H.S.E. Psychology, Counselling in Primary Care, and a range of other services provided through your local health centre.

A Primary Care Team is a team of health professionals who work closely together to meet the needs of the people living in the community. They provide a single point of contact to the health system. The Primary Care Team members also link with other community based services - such as H.S.E. mental health services - to ensure all your health and social needs are supported.
